Rachel Leah Hoppenworth, 28, of Oak Park, died Saturday, December 19, 2015.
She was born February 25, 1987 in Colorado Springs, Colorado to Susan Hyde & Rex Hoppenworth.
Rachel enjoyed spending time with her three sons and watching them play sports.
She is survived by three sons, Jaiden Raymond Hyde, DeMari Anthony Harper, and Marcus Anthony Woolfolk; her mother Susan (John Rudowski) Hyde; her father, Rex Hoppenworth; two brothers, Adrian Hyde-Hoppenworth and Kyle Hoppenworth; several aunts, uncles and cousins. She was preceded in death by her Grandparents Raymond and Margaret Hyde.
Funeral services will be held at 6 p.m. Sunday, December 27, 2015 in Karrer-Simpson Funeral Home with visitation from 12-6 p.m. Pastor Joe Collins of Restoration Station will officiate.
Memorial contributions may be made to the family.
